In routine practice, first-line osimertinib shows robust effectiveness but lower rwOS than expected and substantial post-progression attrition. These findings underscore the need for risk-adapted treatment strategies and support prospective evaluation of OPD management approaches, including integration of LAT alongside contemporary systemic options.

Spectrum of common and uncommon compound epidermal growth factor receptor mutations in non-small cell lung carcinoma: An institutional experience from tertiary care centers from Eastern India. (PubMed, Indian J Pathol Microbiol)
Nearly 7% of EGFR mutations in NSCLC patients were compound mutations, which is comparable to previous reports. The presence of multiple mutations, particularly those involving T790M , may be associated with potential resistance to first-line EGFR -TKIs. We describe a triple mutation involving del19 + G719X + S768I , which represents an uncommon scenario.

Simultaneous evaluation of EGFR, ALK, and PD-L1 in lung adenocarcinomas: the largest single-center experience from southern Brazil. (PubMed, Genet Mol Biol)
Findings on EGFR mutations were consistent with the national and international literature. However, PD-L1 expression rates were higher than those typically reported in Brazilian studies, highlighting regional variation in biomarker prevalence.

Real-World Outcomes and Progression Patterns with First-Line Osimertinib in Patients with Advanced EGFR-Mutant Non-Small Cell Lung Cancer: A Nationwide Turkish Oncology Group Study. (PubMed, Cancers (Basel))
This study supports the real-world effectiveness and tolerability of first-line osimertinib. Oligoprogression was associated with longer OS compared with systemic progression, and the integration of LATs in carefully selected patients with oligoprogression may be associated with prolonged osimertinib treatment duration and favorable post-progression outcomes.

Case Report: Dynamic TKI combination strategies for EGFR-mutant NSCLC with acquired ROS1 fusion and brain metastases. (PubMed, Front Oncol)
We report the case of a 61-year-old woman diagnosed with stage IV lung adenocarcinoma with brain metastasis harboring an EGFR exon 19 deletion (p.E746_A750del), who acquired resistance to osimertinib through a ROS1 fusion bypass pathway. The patient achieved a survival of over 6.5 years from initial diagnosis through ongoing adjustments to targeted therapy, including sequential treatment with crizotinib, entrectinib, and lorlatinib...Notably, we observed an interesting phenomenon with both crizotinib and entrectinib: while initial treatment led to intolerable adverse reactions requiring discontinuation, subsequent reintroduction of the same agent was well-tolerated. This case report aims to provide potential treatment strategies for patients with similar complex co-mutations.

Characteristics and Clinical Outcomes of De Novo and Acquired Uncommon Compound EGFR Mutations in Patients With Advanced Non-Small Cell Lung Cancer: A Brief Report. (PubMed, Clin Lung Cancer)
Our findings demonstrate that compound EGFR mutations comprise distinct subgroups with unique clinical characteristics and different responses to targeted therapy. For example, osimertinib may be a viable treatment for patients with compound G719X/L861Q mutations. Further characterization of these heterogeneous subgroups is warranted for identifying optimal treatment strategies as novel targeted agents emerge.

Endonuclease-Assisted Selective Exponential Amplification (ESEA) for Ultra-Sensitive Enrichment and Detection of Low-abundance Mutant Alleles in Lung Cancer. (PubMed, J Mol Diagn)
In a small-sample-size test, the ESEA system achieved 100% sensitivity and specificity in pleural effusion samples (n=5) and a 100% ctDNA detection rate in patients with extracranial lesions and disease progression (n=6). These results highlight its potential as a cost-effective, highly sensitive, and robust platform for dynamic, real-time companion diagnostics, as well as non-invasive monitoring of treatment response and tumor evolution.
